Qualifications

~ Education: Master of Social Work (MSW) from an accredited institution
~ Licensure: Active New York State LCSW license and active Medicaid enrollment
~ Experience: Minimum of 2 years of clinical social work experience
~ Language: Spanish preferred
~ Technical Skills: Proficiency in standard clinical documentation and telehealth platforms
~ Soft Skills: Strong interpersonal, communication, and empathetic skills; bilingual (Spanish) highly preferred

Key Responsibilities

~ Conduct psychosocial assessments and provide evidence-based therapeutic interventions
~ Develop and implement individualized treatment plans
~ Provide crisis intervention and supportive counseling to patients and families
~ Collaborate with a multi-disciplinary healthcare team to ensure comprehensive patient care
~ Maintain timely and accurate clinical records in compliance with regulations
